FreedomWorks Resident Discipleship Coven= ant Agreement

The purpose of this covenant is to help you, the resident, grow cl= oser to God through His Son, Jesus Christ. Through that relationship, if you will worship Him with your life, you will experience peace, joy and contentment = that can only come from knowing Him. We as staff want you to gain financial independence, making it possible for you to support yourself financially wh= en your stay at FreedomWorks is complete. We also want to help you develop the disciplines that will enable you to live an abundant life in Christ.

I agree to reside at Freedom= Works for at least six months unless the= lease is terminated prior to that time in accordance with the terms stated in the= covenant agreement and Minnesota Multi Housing Association residential lease. I also= agree to notify a FreedomWorks staff member about my plans to move out at least thirty days prior to the planned move-out date. Initials ____

I agree to pay $375/$400 per= month (amount varies b= y size of the unit) du= ring my stay at FreedomWorks, starting with the second month of my stay. (If I do not complete the 6-month minimum stay at FreedomWorks, I will forfeit my security deposit to cover the cost of the first month’s rent that had been waived). Initials ____

I agree to provide a urine analysis (UA) upon entering FreedomWorks’ Residential Discipleship program. I also agree to provide UA’s at staff’s request. (UA&#= 8217;s must be given within one hour of request or it will be considered positive.= A positive UA will result in immedi= ate eviction. Once a UA has been requested, residents cannot leave the main flo= or until the UA has been provided). Initials ____

I agree not to use any form = of tobacco, drink beverages containing alcohol (beer, wine, or spirits) or use= any non-prescription drugs at any time while I’m a resident at Freedom Works. Initials ____

I agree to inform FreedomWor= ks Staff of any medications that I have been prescribed before and during my s= tay at FreedomWorks. I also agree to sign a release of medical information prio= r to moving in to the FreedomWorks apartment. Initials ____

I agree to make FreedomWorks= my home and will be in the FW building by 12 am every night during my stay with FreedomWorks. I further agree= to get permission from the Program Manager before making any commitments to overnight elsewhere. Initials ____

I understand that female gue= sts are not allowed in residents’ apartments at Freedom Works. Other gues= ts are only allowed for half-hour visits, and only with roommates’ and staff’s approval. All visiting will be held in the Welcome Back Center. Only Freedo= mWorks residents are allowed to stay overnight. Initials ____

I agree to comply with all requirements for Phases 1 through 4. I understand that failure to comply with these requirements will res= ult in eviction. Initials _____

I agree to be accountable to= my assigned mentor while I reside at FreedomWorks. I understand that the mentor will help me identify goals, set up and maintain a budget, and discuss decisions that will im= pact my life. Initials ____

I agree to perform volunteer= /service work during my stay at FreedomWorks. FW staff will assist in arranging t= he volunteer opportunities. Initials ____

I agree to share in the duti= es of the care and maintenance of the Welcome Back Center and FreedomWorks. I will be accountable to the FreedomWorks staff for my wo= rk assignments. Initials ____

I agree to participate in Mondays Bible studies, Tuesdays (AV) support groups and/or including chemical dependency/recovery groups, Thursd= ays Fellowship Dinner. Initials __= __
